Establish and maintain positive relationships with clinicians, referring physicians, patients, transplant team, dialysis centers, and transplant center affiliate partners to provide education, assistance, and information for timely referrals across the organ transplant continuum. Maintain and grow existing external partnerships in order to support the financial needs of the UK Transplant Center and UKHC.
Develop and maintain collaborative relationships and communication tools with external care teams to ensure continuity of care. Works in collaboration with UK Physician Liaison Program to monitor and coordinate relationships with referring providers and dialysis centers throughout the state and region and to make routine visits to current and potential referral sources to establish relationships with staff.
