Clinical Facilitators work as part of the Care Redesign Program. This position will serve as a clinical expert to the program, managing multiple projects with bandwidth to run a design and implementation team.
This position assumes direct responsibility for all deliverables and weekly monitoring (both qualitative and quantitative) of initiative core teams; functions independently and provides key updates/information to initiative leaders and teams through both informal and formal communication channels; assists teams with project work that includes any or all of the following: evidence review, data collection, engagement of all providers, meeting coordination and facilitation, verbal and written communications regarding team progress.
